The Florida Department of Health (FDOH) has implemented Florida Health Connect, a patient portal, that provides clients access to their personal health record. Clients use Florida Health Connect to manage all their health information, from all clinicians, in one place. The portal enables individuals to easily and securely collect, store, manage and use their personal health records and wellness information on a voluntary basis. An individual’s personal health information is only accessible by the individual client or those they designate.

Florida Health Connect is a patient portal that provides clients access to their personal health record. The portal is an untethered personal health record, which means it isn’t offered by a health provider or health insurer but is created for your use and under your control. This is important if you get care outside your primary care network, move, or change health plans, because your health information can go with you. Second, Florida Health Connect is there for your access only and you control how your information is used and who has access to it.

It is a private and secure place to collect, store, use, and manage your health information. The online personal health record helps you gather and use all your health information. Using Florida Health Connect, you can easily collect data from doctors, hospitals, and labs, or add data of your own, communicate securely with clinicians at the county health department, and use all the information to manage your health.

What is a personal health record or PHR?

A personal health record is a summary of health-related information – from sources such as physicians, hospitals, labs, or devices – that is solely controlled by the individual user/patient or a representative of a patient with the authority to access such data. Such access by a representative may include but not be limited to those granted permission by the patient that controls the record.

An EHR, or Electronic Health Record, is a digital collection of patient health information compiled at one or more visits in any care delivery setting.
