- You must have a commercial insurance plan that covers Zepbound.
- You must be a resident of the United States or Puerto Rico.
- You must have a valid prescription for Zepbound for an FDA-approved use (chronic weight management).
- You cannot be enrolled in any government-funded healthcare program, such as Medicare, Medicaid, TRICARE, or any other federal or state program.

- The only way to get a legitimate savings card is by visiting the official Zepbound website from Eli Lilly.
Fill Out the Form:
- You will need to fill out a form with your personal and insurance information to verify your eligibility.
Receive the Card:
- Once your information is verified, you will be able to download and print the savings card, or you can have it sent to your email. You can then provide this card to your pharmacy.
Important: The Zepbound Savings Card cannot be used by those with government-funded insurance. For those with no insurance, the card is not applicable, and the medication will be at full cash price, which can be over $1,000 per month.
