Only the nine-digit Illinois State Student ID will be accepted for survey login. Using any other student id will result in an unsuccessful login. State Student IDs are listed in the Survey Admin Dashboard; please see your teacher or building survey coordinator for more information.

Additionally, students should only use the provisional login method if they ARE NOT listed on the roster in the Survey Admin Dashboard. Entire classrooms should not be taking the provisional survey. Improper provisional survey use can result in incorrect final response rates. Provisional surveys will count but are not reconciled until after the survey window closes.

Survey Log In Procedures

Teachers

Teachers log in using their login and passcode. This information is emailed to each teacher at the beginning of the survey administration window.

I can’t find my passcode.

Teachers who lose their passcode can access them by clicking on the “Forgot your passcode? Click here” link on the login page, just underneath the Passcode box. You will be taken to the “lost passcode” page. After you enter your email address, you should receive an email with the passcode information. Each time a teacher requests a new passcode, the program will generate a new passcode. Old passcodes won’t work once a new one is generated.

What if I don’t get a new passcode?

First, check your junk mail folder. Second, contact customer support at 866.440.1874 or im************@uc******.edu to verify that you are on your school roster. If you aren’t on the roster, you can take the survey using a provisional login.

Provisional login

On the lost passcode page, you will see a link that says “Can’t log in? Click here to take the provisional survey.” You will be asked to enter some information to help us verify that you are eligible to take the survey, then click the “login” button. This information will ONLY be used to verify that you are a teacher at your school. We will never use this information to link survey responses to a particular teacher.

Students

Students log in using their student ID and their date of birth. Students who don’t have a student ID yet or have other problems logging into the system can log in provisionally. Underneath the login page for students, click the link that says “Click here to take the provisional survey”. Like the provisional login for teachers, students will be asked questions that will help us identify if this student belongs to this school.

From the survey administration dashboard, you can view and download the student roster to assist the student log in process.

Teacher Provisional Login

Step 1 

Go to your survey homepage and click on the “Forgot your passcode? Click here” link (see image below):

Step 2

If you would like to retrieve your passcode, enter your email address and click “Submit.” If you don’t receive a passcode, click on the link at the bottom of the page (shown with an arrow below) to take the provisional survey.

Step 3

Fill out all the information on the provisional login page, tell us why you need the provisional login, then click “Login” at the bottom of the page. 

Step 4: Take the survey!

Now that you’ve logged in, complete the survey. Remember, you can skip any question if it isn’t applicable to your work, or you don’t feel comfortable answering. Otherwise, we hope you complete the survey. Your principal and district/charter officials will never see your responses.
