Fraud by doctors, pharmacies, or home-health agencies

🕵️ Tips for Anonymous Reporting
Provide as many details as possible: names, addresses, dates, provider numbers, what you saw or heard.
Don’t use work or personal email if you want to stay fully anonymous — use the online form or call.
You may get a report ID number so you can check back without revealing your identity.
