You can activate your Freedom Health Care OTC Flex card by login on to freedom.nationsbenefits.com and enter the 16 digit card number and the expiry date on the card.

FLEX ACCOUNT – ACTIVE FITNESS

✓ Flex Account – Active Fitness: For certain plans, the plan covers a spending allowance of $500 per year towards the payment of facility access fees for golf, tennis, or swimming. Any unused amounts do not carry forward to the next calendar year

2. Enter the 16 digit card number and expiration date of your Benefits Mastercard® Prepaid Card.

3. Your 12-digit member ID contains 2 letters and 10 numbers.

4. Enter your Date of Birth and Last Name provided to your health plan during enrollment.

5. Click on Activate Card Button to activate your card.
