To activate CommunityCare Medicare card, login into ccok.nationsbenefits.com/activate and enter the 16 digit card number and expiration date of your Benefits Mastercard® Prepaid Card.
The Over-the-Counter (OTC) benefit is an additional benefit for most of our plans. CommunityCare Options is committed to providing this program to you as a benefit to help you live a healthy lifestyle and improve the quality of your life. CommunityCare Options Gold has an $85 quarterly Over the County benefit.
- CommunityCare Options Gold has a $50 quarterly Over the Counter benefit.

ccok.nationsbenefits.com Activate Card :
Once you receive and activate your card, you can start using it online through the MyBenefits portal and the MyBenefits app and/or in person at a participating retail location near you. When at a retail location, simply swipe your card for payment
1. Log in to your personalized MyBenefits portal at ccok.nationsbenefits.com/activate or call 833-899-0614 (TTY: 711)
2. Enter the 16 digit card number and expiration date of your Benefits Mastercard® Prepaid Card.
3. Your 12-digit member ID contains 2 letters and 10 numbers.
4. Enter your Date of Birth and Last Name provided to your health plan during enrollment.
5. Click on Activate Card Button to activate your card.
How do I order?
Unused OTC dollars do not roll over to the next quarter. You may spend above your quarterly amount if you want and pay out of pocket for the difference.

When placing your first order, you will need to Register. Enter your member ID with no dashes (example: C00xxxxxxxx). If you are having trouble creating an account, call NationsOTC at 1-833-899-0614 .
By app: To place an order through the MyBenefits app search “MyBenefits” in the App Store or Google Play.
- If this is your first time placing an order via the app or online, you will need to create an account by registering on the MyBenefits portal or the MyBenefits app. If you already created an account, simply log in and select the items you would like to order. Once you’re ready to place your order, click “Checkout.”

Please Note: Order forms should be mailed before the 20th of the month to ensure the order total is applied to the current benefit period. We encourage you to place an order online or by phone if the end of your benefit period is approaching. Please do not send cash or checks in the mail.
