What is PCMH?

Whether you’re trying to get healthy or stay healthy, you need the best care. That’s why CareFirst has created a program to improve health care quality and help slow rising health care costs over time. The focus is on you and your health.

Our Patient-Centered Medical Home (PCMH) Program focuses on the relationship between you and your primary care provider (PCP) so your PCP has a more complete view of your health needs and of the care you’re receiving from other providers.

Extra Care for Certain Health Issues

When you participate in PCMH, your PCP will take specific steps to coordinate and manage your care. If you have certain health issues, your PCP will create an online record of your health needs with specific follow-up activities.

If you have a chronic condition — such as Diabetes, Coronary Artery Disease, Hypertension — or are at risk for one, your PCP may:

  • Create a Care Plan based on your health needs with specific follow-up activities to help you manage your health
  • Provide access to a care coordinator, who is a registered nurse (RN), so you have the support you need, answers to your questions and information about your care

Online payment

    • Set up recurring monthly payments with your checking account or credit/debit card
    • Make a one-time payment with your checking account or credit/debit card
    • View and pay your bill online 24 hours a day, seven days a week using a smartphone, tablet or desktop computer through My Account or the CareFirst mobile app
    • Check the status of your payment and any outstanding balances
    • Enroll in My Account to pay online

Phone payment

    • Make one-time payments using ACH bank account, or credit/debit cards. It’s safe and secure, and your data is always protected.
    • Save your payment information for fast, easy bill pay month after month. It’s free with no late fees.
    • Register with My Account to see your activity and manage your payment and billing options

Cash payment

    • Visit the customer service desk at your local area Walmart, an authorized payment agent of CheckFreePay Corporation
    • Find the location nearest you

    • Present your CareFirst invoice/account number (located in the upper right corner of your bill)
