How to Enroll
Learn how to apply, enroll and choose Anthem Medicaid.
1. Apply.
Visit Benefind to apply for Medicaid or get help. Or call the Department of Community Based Services at 1-855-306-8959. You can also visit your local DCBS – see which DCBS is closest to you.
2. Choose.
After you’re approved, choose Anthem Medicaid as your health plan:
If you apply and are approved using Benefind, you can pick your plan right away on the website.
If you mail in an application, the state will send you a letter to tell you if you can get Medicaid and how you can choose a health plan.
3. Get care.
We’ll send you your welcome packet and member ID card in the mail.
How to renew
1. Keep an eye out for a notice in the mail.
You’ll get a renewal notice in the mail from the state telling you it’s time to renew.
2. Go to the Benefind website
Choose “Let’s Get Started” to log in or create an account.
3. Complete your online renewal.
