If you are trying to complete the Altar of Reflection: Catalyst mission in Destiny 2 (usually for the “Report: ALTAR-REFLECT” quest or to get your Exotic Glaive pattern), “not working” usually means one of two things: either the boss is bugged/immune, or you cannot find the entrance.
Here is how to fix the specific issue you are facing.
1. The “Immune Boss” Bug (Most Common Issue)
The Problem: You solve the rune puzzles, enter the boss room, kill the Hive Lightbearers (Knights/Wizards), but the big Shrieker boss (Witch’s Eye) stays closed and immune forever.
The Fix: This is a scripting error that happens if you kill the “Archivist” enemies too quickly or if the game fails to register the death of the Knight/Wizard minions.
Wipe or Restart: You cannot fix this mid-fight. You must restart the activity.
Slow Down: When you reach the boss room again, kill the red-bar adds first. When the Hive Guardians (Lightbearers) spawn, damage them slowly.
Finish the Ghost: Make sure you crush the Ghost of the Hive Lightbearer immediately. If the Ghost disappears on its own or glitches out, the boss phase will not trigger.
Use a Different Weapon: Some players report that killing the Lightbearers with damage-over-time (like Witherhoard or heavy ability spam) can cause the script to break. Try using a standard Primary or Special weapon for the final blow.

The Fix: The Altars rotate daily. There are four versions: Catalyst, Insight, Pact, and Choice.
Check the Daily Rotation: Look for the generic Altar of Reflection icon on the Throne World map (it looks like a white Hive rune). Hover over it to see which version is active today.
If today is NOT “Catalyst,” you usually have to wait for the daily reset (10 AM PST / 1 PM EST) for it to rotate back.
Force the Location: If you are on the specific Quest step, the game should show you the flag, but it might be hidden in a Lost Sector.
Check “Metamorphosis” Lost Sector (Miasma): The entrance is often hidden inside here. Run through the Lost Sector until you see a Deepsight node (green energy).

The Problem: You are at the location, you see the white marker, but there is no door/portal.

Align the Image: Look for the glowing white lines painted on the walls/rocks. You must stand in a very specific spot so that the lines line up to form the Witch Queen logo (the Savathûn symbol).
Wait for the Sound: Once aligned, the symbol will flash, a sound will play, and the orange portal will open.
